Print and complete the appropriate New Client Intake Forms (turn on 2-sided printing on your printer):
If you are going to attend your sessions via Teleconference, please read, sign, and return the Informed consent Form below.
Once you have completed these forms, you can drop them off at my office or bring them along with your current insurance card to your first appointment. Military personnel are encouraged to bring their DD214.
***IMPORTANT***If you have medical records from a current/previous provider that you feel would better inform our work together, please share my FAX number with your provider, which is: 507-540-1451. Providing me with a current Diagnostic Assessment (Adults - past year; Adolescent - past 6 mos), can save you time and money!
