To prepare for your first appointment please follow these instructions:

​Print and complete the appropriate New Client Intake Forms (turn on 2-sided printing on your printer):  ​
Adult (2-sided, 5 pages)
Adolescent (2-sided, 5 pages)
If you are going to attend your sessions via Teleconference, please read, sign, and return the Informed consent Form below.
Informed Consent for Teleconferencing
Once you have completed these forms, you can drop them off at my office or bring them along with your current insurance card to your first appointment. Military personnel are encouraged to bring their DD214.

***IMPORTANT***If you have medical records from a current/previous provider that you feel would better inform our work together, please share my FAX number with your provider, which is: ​507-540-1451. Providing me with a current Diagnostic Assessment (Adults - past year; Adolescent - past 6 mos), can save you time and money!

Payment Options
1. Fee for service - Please see the Financial Information page in the New Client Intake Form for rates of service.
2. Sliding fee scale - Available in the event that you do not have health insurance or this provider is not covered by your plan.
3. Insurance - You are financially responsible for any services not covered by your insurance, including co-payments, co-insurance, and deductibles. It is important that you contact your insurance company about coverage for services. Any applicable deductible amounts or co-payments are due at the beginning of each appointment. To find out more information about your health insurance coverage, please click on the appropriate link below. ***Please bring your insurance card to your initial session.
